Yuvo Health partners with Particle Health to better inform patient care for community health center support

NEW YORK (July 17, 2024) – Yuvo Health, which partners with community health centers to help them gain an advantage in value-based care (VBC), today announced a partnership with Particle Health, the data-transformation platform for healthcare, to bring more complete health information to the providers served by Yuvo Health. The collaboration allows for faster and fully informed patient care as well as reducing the risk for patient readmissions.

Yuvo Health’s customers are community health centers that have been designated as Federally Qualified Healthcare Centers (FQHCs), which provide primary-care services to medically underserved areas and populations. Particle Health works across the healthcare system to bring providers secure, rapid, and affordable interoperability access to health data and insights, so they can spend more quality time with patients and less time seeking information and preparing reports.
