Yuvo Health partners with Particle Health to better inform patient care for community health center support

Particle Health’s platform will give Yuvo Health’s provider-partners a full picture of each patient’s health, providing context from previous care encounters and generating actionable insights.

NEW YORK (July 17, 2024)Yuvo Health, which partners with community health centers to help them gain an advantage in value-based care (VBC), today announced a partnership with Particle Health, the data-transformation platform for healthcare, to bring more complete health information to the providers served by Yuvo Health. The collaboration allows for faster and fully informed patient care as well as reducing the risk for patient readmissions.

Yuvo Health’s customers are community health centers that have been designated as Federally Qualified Healthcare Centers (FQHCs), which provide primary-care services to medically underserved areas and populations. Particle Health works across the healthcare system to bring providers secure, rapid, and affordable interoperability access to health data and insights, so they can spend more quality time with patients and less time seeking information and preparing reports.

As health institutions continue to transition to VBC models, especially those working with at-risk patient populations, many providers cannot instantly access patient data to proactively treat patients’ evolving needs. Particle Health’s ability to deliver real-time patient insights, regardless of where care is delivered, enables FQHC providers to intervene as necessary for ensuring quality outcomes and VBC performance.
